APPLICATION FOR A NATIONAL VOLUNTEER
POLICE CHECK

The Application Form is to be completed in full and
forwarded to the District Registrar for referral to the Police for
processing. Upon receival of the Police Response, the District
Registrar shall advise the applicant of the outcome of the response
and place same on a District Register.
Provided applications are made under the auspices of Lions
District 201W2 and provided by club Secretaries to the District
Registrar for processing, the cost of each application is $10. The
member’s club can pay this fee from its Activity Account, as
the application is to support club projects. A club cheque to cover
the cost of each application/s, made in favour of “District
201W2”, is to be forwarded to the District Registrar with any
application/s.
Part C of the Application Form requires verification of
the identity of the member requesting the application. This can be
provided by an executive member of the club, usually the club
Secretary. It is not necessary to forward to the District Registrar
copies of applicant identification documents.
Part C also requires that the information received from a
Police Response to an application is treated in confidence and used
only for the sole purpose of screening volunteers.
The Response to an application is considered to be the
property of the applicant. The applicant’s name is to be
added to a Club Register of members having obtained a National
Volunteer Police Check when the applicant provides to the club
officer designated to manage Police Checks evidence of having
obtained a Police Check that permits the applicant to participate
in club projects the subject of the Act.
Under the Act, an applicant is disqualified from
participating in projects and activities involving working with
Children (young people under eighteen), working with Seniors and
working with People with Disabilities if the response to an
application for a National Volunteer Police Check advises that the
applicant has a disclosable Police record thus:
“S, The person has serious criminal conviction/s.
(That is, a criminal conviction for any sexual offence or any
offence that was heard in the District or Supreme Court of Western
Australia, i.e – Higher Court).
